Early Intervention Service Coordinator (Full-time)
The Service Coordinator (SC) will work with families with children ages birth-3 who have documented developmental delays or conditions likely to lead to delays (e.g. Autism, cerebral palsy, Down Syndrome, brain injuries). The SC will assist families with eligibility, coordinating services, seeking services necessary to benefit the development of each child, facilitating and assisting in development of Individualized Family Service Plans (IFSPs); maintaining appropriate paperwork; and meeting with families via video-conferencing and/or home visits as needed. Bachelor’s Degree in human service field (child development, social work, psychology, education, rehabilitation, sociology, etc.) and professional experience working with children and/or families with disabilities required.
